DOH PMP-EHR Integration and E-Prescribing Mandate Waiver Attestation Forms

The Department of Health (DOH) issued the waiver attestation forms for the Prescription Monitoring Program (PMP)-Electronic Health Record (EHR) integration mandate and the electronic prescribing mandate that took effect as part of SB 5380 (2019). The PMP-EHR integration mandate applies to facilities, entities, offices, or provider groups with ten or more prescribers, that are not critical access hospitals. The compliance deadline for the integration mandate was extended to Jan. 1, 2022 due to the COVID-19 pandemic.

The electronic prescribing mandate requires all health care practitioners to communicate prescriptions and prescription refills for Schedule II-V controlled substances to the pharmacy electronically. Both waiver attestations last for one year and may be renewed two times for a total of three waivers, except in the case of other exceptional circumstance waivers. Other exceptional circumstance waivers may be submitted an unlimited number of times. Access information about the two waiver attestation forms online here.
