Using the EHR “SDOH Wheel” as a tool for screening & referring patients with health-related social needs

Many hospitals have integrated a process to screen patients to identify health-related social needs, a key determinant of health. Social determinants of health (SDOH) are estimated to account for at least 80% of health outcomes. EHR systems are beginning to make the process of screening and documenting SDOH (such as housing instability, food insecurity, transportation, utilities, and interpersonal violence) faster and easier. One application gaining popularity as a tool for screening and referring patients is the “SDOH Wheel.” Hospital members using or with access to such a tool in their EHR system are invited to attend this discussion to learn more from each other about how it can be used to enhance screening, documenting and improving care for patients.
