Tool available for responding to community health needs assessment

Hospitals and health systems want to make an impact when responding to community health needs assessments, but searching for examples of successful community health interventions can take tremendous time and energy. The CDC Community Health Improvement Navigator brings together tools, resources and examples to help hospitals and health systems get started. The Database of Interventions helps identify interventions that work in four priority action areas: socioeconomic factors, physical environment, health behaviors and clinical care.

The database includes filters to limit a search by target risk factors, target populations, target outcomes or indicators, intervention settings/locations, intervention types, and assets. This comprehensive resource provides hospitals and health systems of all sizes with trusted tools and information to begin developing a community health improvement plan. (Lisa Segerstrom)
