Medical errors are the third leading cause of death in the United States. By sharing our safety events, other hospitals don’t have to experience an event to learn about risk in our systems and can instead proactively take steps to mitigate potential problems.
WSHA is uniquely positioned to provide a confidential forum to all its members under its Coordinated Quality Improvement Program (CQIP). WSHA launched Patient Safety Alert Huddles in 2017. Safety events are presented by hospitals in SBAR format followed by discussion facilitated by WSHA staff. An event summary and any tools and resources shared are sent to participants after the call.
Below are testimonials from our participating organizations and participants.
“Patient Safety Alert Huddles are an opportunity to connect fellow quality leaders and know we’re not alone in managing safety events.”
“WSHA provides an open, safe environment to ask for help.”
“Sharing best practices from serious events and good catches.”
“Participating in WSHA Patient Safety Alert Huddles has enhanced our own safety huddles.”
