As we shared last week, WSHA is offering an upcoming collaborative to assist hospitals in improving systems to reduce diagnostic errors. The WSHA Journey to Diagnostic Excellence collaborative will assist hospitals in assessing their current state of diagnostic processes, identify opportunities for improvement, and tracking their progress as they implement improvement efforts. Diagnostic errors include delayed, incorrect and missed diagnoses. According to the Improving Diagnosis in Health Care report, a diagnostic error will affect each of us in our lifetime. It is also estimated that diagnostic errors account for 40,000-80,000 deaths per year in U.S. hospitals.
