Honoring Choices Pacific Northwest held a kickoff event Oct. 23 for teams that will begin implementing advance care planning in their organizations. More than 110 participants from across Washington State came together to learn more about the first steps needed to program implementation. The groups in this first cohort include hospitals, health systems and medical groups of all sizes, representing all areas of the state.
This kickoff event marked the beginning of a 12+ month training and consultation process that provides advance care planning program design and implementation strategies that participants can take back and immediately apply in their organizations.
A joint initiative of the Washington State Hospital Association and the Washington State Medical Association, Honoring Choices Pacific Northwest will provide health care teams with training and resources, and patients will also get the tools they need to make informed care choices and talk to their families and health care providers about their end-of-life wishes.Click here for more information.
