Access to housing is one of the most common barriers to discharge from the inpatient setting. Unfortunately, this often leads to patients being discharged back onto the streets without adequate housing. The Washington State Healthcare Authority (HCA) has created the Discharge Planner Toolkit to support discharge planners, case managers and other staff in connecting patients with appropriate housing resources in their area. The toolkit provides both an electronic decision tree as well as a comprehensive list of available resources. The electronic decision tree easily allows staff to identify resources appropriate for the individual patient based on their specific circumstances.
