The Triple Aim of health care is to improve the patient experience, improve the health of the population and make health care cheaper per-capita. The new report addresses how hospitals can develop new delivery systems and innovative payment models that best meet the needs of their organizations and communities, as new delivery systems will have to be woven into the fabric of local communities and lives of patients and families.
