As follow up to the webcast held on September 23, the next phase of the Washington State Hospital Association's (WSHA) collaborative effort to examine nurse staffing is about to begin. Part of the Memorandum of Agreement WSHA and Northwest Organization of Nurse Executives (NWONE) signed with the nursing unions, was to encourage all hospitals to report data on five selected nurse sensitive quality indicators. Five indicators were selected: patient falls, patient falls with injury, pressure ulcers, and two staffing indicators. Beginning in October, all Washington hospitals will be asked to report this data.

This webcast for project leads, data entry staff, and other system users will discuss the technical aspects of reporting and using the system.

Overview of Nurse Sensitive Quality Indicators (NSQI) Project

September 23, 2008 -- Noon - 1:00 p.m.

The next phase of the Washington State Hospital Association's (WSHA) collaborative effort to examine nurse staffing is about to begin. As part of the Memorandum of Agreement WSHA and Northwest Organization of Nurse Executives (NWONE) signed with the nursing unions, we agreed to encourage all hospitals to report data on five selected nurse sensitive quality indicators. After surveying hospitals to determine what measures hospitals are already collecting, the steering committee overseeing the agreement selected five: patient falls, patient falls with injury, pressure ulcers, and two staffing indicators. Beginning in October, all Washington hospitals will be asked to report this data.  

 WSHA will hold a webcast on September 23 from 12:00 to 1:00 p.m. for Chief Executive Officers, Chief Nursing Officers, and Chief Financial Officers to provide additional project information, discuss the reporting tool, answer any questions, and ask hospitals to enroll.

Medicare Linkage: Quality and Payment

July 28, 2008 -- 10 - 11:30 a.m.

The presentation focuses on the inpatient and outpatient payment changes proposed by the Centers for Medicare and Medicaid Services (CMS), with a particular emphasis on Value Based Purchasing. If approved by Congress, Value Based Purchasing has the potential for significant impact on hospital payments, as up to 5% of your prospective payments might be withheld from market basket payment. Hospitals will have to earn back these dollars based upon quality results. This web seminar will also cover other proposed Medicare payment changes.As this session is designed to help answer questions posed by CFOs and Quality Leaders, we would strongly encourage staff to take this call together.

    Washington State Living Will Registry

    The 2006 Washington legislature passed a bill to create an electronic registry for healthcare directives to facilitate end of life planning. he registry is now in full operation and individuals have begun storing their documents in the registry. Hospitals can expect to see an increasing number of patients who have stored documents in the registry and wish their care givers to access the documents as part of their care. Carol Wozniak, Living Will Registry Manager, Washington State Department of Health discusses how individuals deposit their documents in the registry and how providers can access an individual's directory documents with proper authorization.

    Protecting Health Information and Responding to Data Thefts 

    In recent months, information about thousands of patients in Washington and elsewhere has been stolen from hospitals or has been reported missing. These incidents have resulted in criminal prosecutions, investigations by the federal Office for Civil Rights, and private lawsuits. The costs of investigation, remediation and defense are extraordinary. (Dec. 11, 2006)




    Proposed Changes to Medicaid Hospital Rates

    In 2005, the Washington State Legislature instructed the Health and Recovery Services Administration (HRSA) to study and make recommendations on the Medicaid inpatient payment system. HRSA hired Navigant Consulting Inc. to perform this analysis. Navigant has finished its work, and is recommending significant changes to the Medicaid payment system. The effect on some hospitals will be significant.

    Specialty Hospitals Mean Special Challenges

    Community hospitals have had concerns over specialty hospitals for years. In early 2003, an application for a Certificate of Need to build a specialty hospital in Tacoma was filed. WSHA and its members fought for legislative policy to prevent the building of specialty hospitals, winning a one-year moratorium in the 2005 legislative session. That moratorium expired July 1.

    Some Washington hospital CEOs and boards are nearly certain that key groups of physicians will soon file for a CON to build specialty hospitals. For many hospitals, such a development will be like the Pontiac Division of General Motors saying, "We're out of here. We are taking our revenue and profits and going it alone. Good luck without us."

    Doug Ross and Ed Rauzi from Davis Wright Tremaine law firm give members guidance on what hospitals can do (and what they can't) if they are faced with such news.

    Quality and Reimbursement Now Linked: New Medicare Rule and the Impact Quality Reporting Will Have on Payment

    Roundtable Web Conference

    The proposed Medicare inpatiend payment rule expands the number of measures that need to be submitted from ten to twenty-one, including measures for surgical infection. If hospitals do not provide the expanded data set, they could lose two percent of their Medicare payments. The validity of the hospital's abstraction process will also become critical to maintaining full Medicare payment. This web conference addresses the requrements in the Medicare rule for prospective payment, the criteria for getting the full payments, and what the abstraction validity reports mean. It also addresses what you should be expecting from your data vendors now and in the future in order to meet these requirements.

    Medication Reconciliation

    One of the six interventions of the IHI 100K Lives Campaign is to reduce adverse drug interactions through medication reconciliation. It is also a requirement for JCAHO 2006 as a patient safety goal. In this presentation you will hear from Rick Croteau, Executive Director of Patient Safety Initiatives for JCAHO, about the best practices they have seen as they have traveled around the country surveying hospitals on medication reconciliation.

    Hospitals face increasing questions from the media and the public about patient safety and quality. Our statewide poll told us that while the image of hospitals is overwhelmingly positive, Washingtonians want more information about what hospitals are doing regarding patient safety and quality.     Medication Reconciliation: Understanding Regulations & Keys to Successful Implementation

    Medication reconciliation to reduce adverse drug interactions is one of the six interventions of the IHI 100k Lives Campaign. It is also a requirement for JCAHO 2006 as a patient safety goal. Ineffective medication reconciliation causes up to 50% of the medication errors which occur at admission and up to 20% of future adverse drug events. This web conference focuses on: JCAHO medication reconciliation requirements and survey processes, communication strategies for developing support and implementation.

    Washington Hospital Image Public Opinion Poll

    In May, the Washington State Hospital Association (WSHA) and the Association of Washington Public Hospital Districts (AWPHD) conducted a statewide public opinion poll on the image of Washington's hospitals. A representative sample of Washington residents were asked a series of questions about the quality of care available at hospitals in our state. The results are overwhelmingly positive, more than 80 percent of Washingtonians are satisfied with the care they receive at hospitals. This presentation gives an overview of the findings.

    Ventilator-Associated Pneumonia & Central Line-Associated Infection

    Ventilator-Associated Pneumonia (VAP) and Central Line-Associated Infection are two of the most common infections in hospitals. The good news is that these infections can be virtually eliminated through the implementation of a group of care techniques or "bundle". Several hospitals in the state of Washington have already implemented this bundle and have virtually eliminated ventilator-associated pneumonia and central line-associated infection. In this web conference, we highlight the experiences of Overlake Medical Center and Swedish Medical Center and provide information on how to implement these techniques in your hospital.

    Rapid Response Teams

    Rapid Response Teams are one of the most effective measures hospitals can take to reduce preventable mortality in the hospital. Rapid Response Teams are summoned at any time by any staff in the hospital to assist in the care of a patient who appears acutely ill, before the patient has a cardiac arrest or other adverse event.     Surgical Clinical Outcomes - Assessment Program (SCOAP)

    The Washington State Health Care Authority will soon require any hospital providing care to state employees and performing certain general surgical procedures (including colectomies, appendectomies, and bariatric surgery) to participate in the Surgical Clinical Outcomes Assessment Program. This program, patterned after the Clinical Outcomes Assessment Program for cardiac surgery, is designed to improve outcomes of general surgical procedures across the state.     Medical Home Pilot

    Thursday, August 5, 2010 | 3:30 p.m. -4:30 p.m.

    With federal health care reform, there has been a lot of discussion about increased primary care and the notion of medical homes.  Washington State has been promoting medical homes and will soon institute a pilot project.  The goal is to determine if there are improved outcomes and savings in hospital care from better primary care.  The Washington State Health Care Authority, the state Department of Social and Health Services, and the Puget Sound Health Alliance have been working with private payers in the state to develop a Multi-payer Medical Home Reimbursement Pilot.  The state plans to launch the pilots in January 2011, and applications are due by September 3, 2010.  The pilots are designed to reward practices for reducing preventable emergency room and hospital admissions and to enhance primary care.

    The web cast will cover the following:

        * Overview of medical homes;
        * The multi-payer medical home pilot – how to participate and the payment models; and
        * Answer questions about the feasibility of participating in the pilot.

    Speakers for the web cast include WSHA staff, along with Richard Onizuka, Ph.D of the Health Care Authority and Reena Koshy, M.D., MPH, Project Coordinator for the Multi-payer Medical Home Pilot Project at the Puget Sound Health Alliance.

    Medicaid DSH Audit Review

    Monday, August 2, 2010 | Noon-1:30 p.m.

    Clifton Gunderson LLP, the independent auditor contracted by the Health and Recovery Services Administration completed their audit of hospital records and state practices related to the state's Medicaid payments for disproportionate share for State Fiscal Years 2005 and 2006. Under federal law, each state is required to have independent audits done of hospital records and DSH payment completed and submitted to the federal government. The audit process for State Fiscal Year 2007 is expected to begin soon.

    Staff from Clifton Gunderson and Washington's Medicaid Purchasing Administration (formerly Health and Recovery Services Administration) will provide an overview of the audit findings and the process going forward. Learn what you need to know to minimize the potential of unnecessary DSH overpayments and recoupments for the services you are providing.

    The Board's Responsibility for Ethical Policy Making

    Over the years WSHA has presented speakers to assist board members think about the ethical aspects of the tough decisions they must make.  Johnny Cox, former ethicist at Sacred Heart Medical Center and Children’s Hospital, Spokane, presented at an annual meeting many years ago and Fr. William Sullivan, retired President of Seattle University, gave an outstanding speech at the Trustee Breakfast in October 2003.
     
    The challenges of governance are certainly not any less complex in 2010 than they were in 2003.  Surveys of Washington hospital board members indicate continued interest in learning how to think through the ethical aspects of their decision-making.  With that in mind, WSHA invites you to a web cast March 26, 2010.  Please note:  this web cast begins at 7 a.m.

    EHR Incentives for Critical Access Hospitals and Rural Providers

    The American Recovery and Reinvestment Act suggests that there will be incentive payments to hospitals and physicians (and others) that treat Medicare and/or Medicaid patients.

    CEO Update - What You Should Know About Infection Rate Reporting

    Carol Wagner, R.N., Vice President Patient Safety and Antony Marfin, M.D., State Epidemiologist, Communicable Diseases, Washington State Department of Health will present the latest on public reporting of hospital infection rates. Under legislation enacted in Washington State in 2007, hospital-specific infection rates will become public this fall.

     

    The Medicaid DSH Audit - What You Need to Know

    July, 16 2009 8:30 am-Noon

    The Health and Recovery Services Administration recently selected an independent auditor to audit hospital records and state practices related to the state's Medicaid payments for disproportionate share for State Fiscal Years 2005 and 2006. Under federal law, each state is required to have an independent audit done of hospital records and DSH payment competed and submitted to the federal government by December 31. While the audit applies to state practices, hospitals will be required to submit data to substantiate their DSH payments. The contractor selected, Clifton Gunderson LLP, has experience working in, and is contracted to provide the DSH audits in, many other states.

    Out Patient Prospective Payment System

    Join the Health Information Program (HIP) for detailed information learned from the Medicare Outpatient Prospective Payment System and the Center for Medicare and Medicaid Services. During this web cast you'll see the detailed data that is being sent to Washington hospitals and some suggestions on how to use it. Additionally, HIP will roll-out the 2009 version of our state analysis and the optional hospital-specific report that will be available upon request. Together these are the most detailed outpatient data available on outpatient services (albeit only for Medicare) provided by Washington hospitals.

    Nurse Sensitive Outcome Indicators Part I: What They Are, and How They Are Measured and Benchmarked

    Nearly all Washington State hospitals are collecting nurse sensitive quality indicator data, including information on falls, falls with injury, pressure ulcers, nursing hours per patient day and frequently more. These two Web casts are designed to help nurse staffing committees learn about the different nurse sensitive quality indicator metrics and how committees can use the information to inform the committee's work.

    This first session will provide helpful background information about evolution of Nursing Sensitive Quality Indicators. The development and common definitions of key measures will be reviewed as well as how nursing quality measures are being implemented nationally and shaping public policy.

    Governance Education: Workforce Issues

    When Jamie Orlikoff, nationally-known speaker and author on governance issues, addressed the WSHA members at the 2007 Annual Meeting, he recommended that hospital boards conduct so-called "deep dives" on the most important topics boards must face. One of the topics Orlikoff specifically mentioned was workforce issues. 

    To help governing boards to think further about workforce matters, Jaime Garcia and Ed Phippen, WSHA staff members, will present what we know about the present and future health care workforce, what the association's Health Work Force Institute (HWFI) is doing to assist, and suggest some resources boards can use to study the topic more thoroughly and locally.

    Clinical Placement

    The Washington State Hospital Association and the State Board for Community & Technical Colleges invite you to learn more about an innovative solution to scheduling clinical education for nursing students. Several hospitals in Washington are already using the Statewide Centralized Clinical Placement Project (SCCPP) to increase the number of clinical education placements available while reducing the amount of health care hospital staff time devoted to managing placements. The SCCPP also ensures student Joint Commission and safety compliance, supplies a nursing recruitment pool for hiring, and provides the opportunity for nursing education programs to expand capacity.     Safe Patient Handling

    The Washington State Safe Patient Handling Steering Committee will share up-to-date information regarding the 2006 state mandate for implementation of safe patient-handling programs in Washington acute care hospitals.     HEET Grant Web Cast-Preparing to Apply for Health Employee Education and Training Grants

    Initiative 1000, which allows terminally ill adults to request and self-administer lethal medications prescribed by a physician, becomes law March 4, 2009.     2009 Legislative Session

    The 2009 session of the Washington State Legislature will convene on January 12, 2009. Many issues are at stake for hospitals and health care during this session. The biggest one, of course, is the substantial state deficit and the possibility of major cuts to hospitals and health care. Other issues WSHA will be following include infection control, nurse staffing, tax exemptions for non-profit hospitals, and the mental health system.

    When the HIPAA privacy regulation became effective in April of 2003, it changed how hospitals disclose protected health information to law enforcement authorities. As a result of the new intersection of HIPAA and state law, the ability of hospitals to make disclosures to law enforcement has been restricted. In many cases established working relationships between hospitals and law enforcement have become strained.
