WSHA hospitals participate in national readmissions pilot

Patient and caregiver interviews at the time of readmission are an effective way to identify the root cause of readmission. In March, five Washington hospitals participated in a Partnership for Patients Sepsis Readmission Interview Pilot to better identify root causes related to sepsis, which is the leading discharge diagnosis resulting in a high rate of readmissions.

We would like to thank the following hospitals that participated:

  • Confluence Health
  • Kadlec Regional Medical Center
  • PeaceHealth Southwest Medical Center
  • Providence Regional Medical Center Everett
  • Virginia Mason Medical Center

National interview results will be compiled and the pilot findings distributed in June 2018. WSHA will then work with hospitals in our region to test interventions to reduce sepsis readmissions. Thank you for undertaking this important work to reduce the morbidity of sepsis patients!
