Invitation to join diagnostic errors collaborative

WSHA would like to highlight the following limited time opportunity to partner with Society to Improve Diagnosis in Medicine (SIDM) and Constellation on improving the diagnostic process during two of the three key stages of the diagnostic process: tests and results processing and follow-up and coordination. Clinic, hospital or health system QI teams interested in improving the diagnostic process should join.

In the last decade, diagnostic errors are the leading category of paid malpractice claims for US hospitals, reaching about 30 percent or an estimated cumulative cost expected in the $50 billion -100 billion range. Furthermore, it is anticipated that 40,000-80,000 hospital deaths will occur, and 12 million additional people will be affected by diagnostic errors each year. Diagnosis-related claims involving follow-up system failures, analysis reveals that accurate and timely diagnosis depends nearly as much on the health care team and systems as it does on the provide. Re-engineering diagnostic workflows, implementing reliable health information technology systems and boosting communication (closed loop) processes creates stronger care teams, increases efficiency and productivity and improves the diagnostic process.
