STAAR Project: Reducing Rehospitalization

Keeping people healthy after they've been hospitalized is the goal of the Institute for Healthcare Improvement's State Action on Avoidable Rehospitalization, or STAAR project. Washington is one of three states to participate in this exciting pilot project. Through the coordination of care transitions from hospitals to home, the project aims to reduce rehospitalization rates by 30 percent and improve patient satisfaction with care coordination.
