STAAR Project: Reducing Rehospitalization
Keeping people healthy after they've been hospitalized is the goal of the Institute for Healthcare Improvement's State Action on Avoidable Rehospitalization, or STAAR project. Washington is one of three states to participate in this exciting pilot project. Through the coordination of care transitions from hospitals to home, the project aims to reduce rehospitalization rates by 30 percent and improve patient satisfaction with care coordination.

Washington hospitals are participating in a groundbreaking initiative to lower rehospitalization rates. As part of the four-state initiative with the Institute for Healthcare Improvement, Washington hospitals and their association are helping lead the nation.
